Benztropine mesylate is an anticholinergic (antimuscarinic) agent that helps restore the balance between dopamine and acetylcholine in the brain, reducing tremor, rigidity, and muscle spasm associated with Parkinson's disease and drug-induced extrapyramidal symptoms (such as acute dystonic reactions from antipsychotic medications). It is typically dosed at 1 to 2 mg once or twice daily for Parkinsonism, or 1 to 4 mg once or twice daily for drug-induced extrapyramidal reactions, with doses individualized and started low. Common side effects include dry mouth, blurred vision, constipation, and urinary retention. Confusion and hallucinations may occur, particularly in elderly patients, who are more sensitive to anticholinergic effects. Benztropine is contraindicated in narrow-angle glaucoma, myasthenia gravis, and GI or urinary tract obstruction. Because it can reduce sweating, it should be used with caution in hot weather due to increased risk of heat stroke.
