Bumetanide (Bumex) is a loop diuretic that inhibits sodium, potassium, and chloride reabsorption in the loop of Henle, promoting rapid diuresis. It is used to treat edema associated with heart failure, cirrhosis of the liver, and kidney disease, including nephrotic syndrome. Bumetanide is significantly more potent per milligram than furosemide; typical oral doses range from 0.5 mg to 2 mg once or twice daily, with a maximum of 10 mg per day. It is also available as an intravenous injection for patients unable to take oral medication or requiring rapid effect. Common side effects include electrolyte imbalances (low potassium, sodium, or magnesium), dehydration, dizziness, and low blood pressure. High doses, particularly given intravenously, carry a risk of hearing loss or tinnitus (ototoxicity), especially with concurrent use of other ototoxic drugs. Regular monitoring of electrolytes, kidney function, and blood pressure is recommended during treatment.
