Carbidopa/Levodopa combines a decarboxylase inhibitor (carbidopa) with a dopamine precursor (levodopa) to treat Parkinson's disease, Parkinsonism, and restless legs syndrome. Levodopa is converted to dopamine in the brain to replenish depleted striatal dopamine; carbidopa blocks the peripheral breakdown of levodopa, allowing more to reach the brain while reducing nausea and other peripheral side effects. Available as immediate-release, orally disintegrating, and extended-release tablets. Dosing is individualized and typically started at a low dose (e.g., 25 mg/100 mg three times daily) and titrated gradually based on response. Common side effects include nausea, dizziness, dyskinesia (involuntary movements), and orthostatic hypotension. Long-term use may be associated with motor fluctuations ("wearing-off" and "on-off" effects). Abrupt discontinuation should be avoided due to risk of a neuroleptic malignant syndrome-like reaction.

Carbidopa/Levodopa (Sinemet) is used to treat the motor symptoms of Parkinson's disease and Parkinsonism, including tremor, rigidity, and bradykinesia. Carbidopa prevents the peripheral breakdown of levodopa, allowing more levodopa to reach the brain and reducing nausea and vomiting.
Common side effects include nausea, dizziness, dyskinesia (involuntary movements), vivid dreams, and orthostatic hypotension. With long-term use, patients may experience motor fluctuations such as "wearing-off" episodes between doses.
Important safety information: Do not stop this medication abruptly, as rapid discontinuation can cause a syndrome resembling neuroleptic malignant syndrome (fever, muscle rigidity, altered mental status). Levodopa should be used cautiously with MAO inhibitors and may interact with high-protein meals, which can reduce absorption.
