Dorzolamide is a topical carbonic anhydrase inhibitor that reduces aqueous humor production in the eye, lowering intraocular pressure in patients with ocular hypertension or open-angle glaucoma. It is used alone or in combination with other IOP-lowering agents such as timolol. The typical dose is one drop in the affected eye(s) three times daily. Common side effects include burning or stinging upon instillation, blurred vision, and a bitter taste in the mouth. As a sulfonamide, it should be used cautiously in patients with a sulfa allergy, and systemic absorption can rarely cause metabolic acidosis, particularly in patients with renal impairment. If using other topical eye medications, wait at least 10 minutes between applications, and remove contact lenses before instilling (they may be reinserted 15 minutes later).
