Perphenazine (Trilafon) is a first-generation (typical) antipsychotic of the phenothiazine class that blocks dopamine D2 receptors to reduce psychotic symptoms; at lower doses it is also used as an antiemetic for severe nausea and vomiting. It is FDA-approved for the management of schizophrenia and for the control of severe nausea and vomiting in adults. Like other phenothiazines, it carries risks of extrapyramidal symptoms, sedation, and orthostatic hypotension, and carries a boxed warning for increased mortality when used in elderly patients with dementia-related psychosis.
