Risedronate (Actonel) is a bisphosphonate that inhibits osteoclast-mediated bone resorption, helping to preserve bone density and reduce fracture risk. It is FDA-approved for the prevention and treatment of osteoporosis in postmenopausal women and men, as well as for glucocorticoid-induced osteoporosis and Paget's disease of bone. Because oral bisphosphonates are poorly absorbed and can cause esophageal irritation, risedronate must be taken with a full glass of plain water on an empty stomach, and patients must remain upright (sitting or standing) for at least 30 minutes afterward before eating or lying down.
