Sulfamethoxazole/trimethoprim (Bactrim, Septra) combines two antibiotics that block sequential steps in bacterial folic acid synthesis, producing a synergistic antibacterial effect. It is FDA-approved for the treatment of urinary tract infections, certain respiratory infections, traveler's diarrhea, and Pneumocystis jirovecii pneumonia, among other bacterial infections. This combination carries a risk of serious skin reactions (including Stevens-Johnson syndrome), blood dyscrasias, and hyperkalemia, and should be avoided in patients with a sulfa allergy or significant kidney impairment; adequate hydration is recommended during treatment.
