Telmisartan/hydrochlorothiazide (Micardis HCT) combines an angiotensin II receptor blocker (telmisartan), which relaxes blood vessels, with a thiazide diuretic (hydrochlorothiazide), which reduces blood volume through increased sodium and water excretion, providing complementary blood-pressure-lowering effects. It is used when blood pressure is not adequately controlled on either component alone. As with other ARB combinations, it carries a boxed warning against use during pregnancy due to the risk of fetal injury, and periodic monitoring of electrolytes and kidney function is recommended.
