Vienva combines a progestin (levonorgestrel) and a low-dose estrogen (ethinyl estradiol) to prevent pregnancy primarily by suppressing ovulation, in addition to thickening cervical mucus and altering the uterine lining. It is taken once daily in a 28-day cyclic regimen (21 active tablets followed by 7 placebo or low-dose tablets, depending on formulation) to maintain contraceptive effectiveness and produce a regular withdrawal bleed. Combined oral contraceptives carry class warnings for increased risk of blood clots, stroke, and cardiovascular events, particularly in smokers over age 35 or those with other cardiovascular risk factors.
