Zavzpret (zavegepant) is a calcitonin gene-related peptide (CGRP) receptor antagonist, a class of migraine medications known as "gepants." It is delivered as a single-use intranasal spray and is used for the acute treatment of migraine attacks, with or without aura, in adults — it is not approved for migraine prevention. CGRP is a protein released during migraine attacks that promotes blood vessel dilation, neurogenic inflammation, and transmission of pain signals. By blocking the CGRP receptor, zavegepant interrupts this pathway and helps relieve migraine pain and associated symptoms such as nausea, light sensitivity, and sound sensitivity. Zavzpret is administered as one spray (10 mg) into one nostril at the onset of a migraine attack. Only one dose should be used within a 24-hour period; the safety of treating more than a limited number of migraine attacks per month has not been established, and patients should discuss frequency of use with their prescriber. The most common side effect is an altered or unpleasant sense of taste (dysgeusia), which occurs in roughly 1 in 5 patients. Other reported effects include nasal discomfort, nausea, and vomiting. Zavzpret is contraindicated in patients with a known hypersensitivity to zavegepant or any component of the formulation; hypersensitivity reactions, including dyspnea and rash, have been reported. Patients should tell their prescriber about all other CGRP-targeting medications they use, since combining multiple gepants or CGRP monoclonal antibodies is generally not recommended without medical guidance.
