Amlodipine/valsartan is an oral, fixed-dose combination prescription tablet pairing a dihydropyridine calcium channel blocker (CCB) with a non-heterocyclic angiotensin II receptor blocker (ARB).
Clinically, this combination tablet is indicated for the treatment of hypertension. It is utilized within two primary management frameworks:
By combining two distinct mechanisms of action—inhibiting calcium ion influx into vascular smooth muscle and blocking the renin-angiotensin-aldosterone system (RAAS)—the formulation delivers a synergistic reduction in peripheral vascular resistance. Crucially, the combination also mitigates certain treatment-limiting side effects inherent to CCB monotherapy.

Oral film-coated tablets (available strengths: 5 mg/80 mg, 5 mg/160 mg, 10 mg/160 mg, 5 mg/320 mg, and 10 mg/320 mg)
Adults typically take one tablet by mouth once daily, with or without food. The starting dose depends on the patient’s blood pressure control and previous antihypertensive treatment. Dosage may be adjusted after 1–2 weeks if necessary. The maximum recommended dose is 10 mg/320 mg once daily. Tablets should be taken at approximately the same time each day and swallowed whole.
Mechanism
Pharmacodynamics and Vasodilatory Synergy
The two active components target peripheral vascular resistance through completely independent, highly complementary physiological pathways:
A common, dose-dependent side effect of amlodipine monotherapy is the development of peripheral ankle edema. This occurs because CCBs cause potent dilation of the afferent arterioles leading into the capillary beds, without dilating the efferent venules. This uncoupling elevates local capillary hydrostatic pressure, forcing fluid into the surrounding interstitial tissue.
When paired with valsartan, the ARB component induces balanced dilation of both the afferent and efferent vascular beds. This restores homeostatic capillary pressure, significantly reducing the incidence and severity of fluid leakage compared to taking high-dose amlodipine alone.

Side Effects
Boxed Warning: Fetal Toxicity
Drugs that act directly on the renin-angiotensin system can cause serious injury, developmental abnormalities, and death to the developing fetus.
Severe Hypotension and Volume Depletion
In patients with an activated renin-angiotensin system—such as individuals with severe volume depletion or hyponatremia secondary to high-dose diuretic therapy, strict dietary salt restriction, or persistent vomiting and diarrhea—initiating this tablet can trigger symptomatic, acute hypotension, syncope, or myocardial hypoperfusion. Volume and electrolyte deficits must be fully corrected before administering the first dose.
Impaired Renal Function and Accelerated Azotemia
Inhibiting the RAAS pathway can cause predictable declines in renal perfusion pressure. In patients whose renal function relies heavily on the activity of the renin-angiotensin-aldosterone axis (e.g., individuals with severe congestive heart failure or bilateral renal artery stenosis), treatment with valsartan can precipitate acute oliguria, progressive azotemia, and acute kidney injury (AKI). Serum creatinine, blood urea nitrogen (BUN), and potassium levels must be monitored regularly.
Common Adverse Reactions
The most frequent side effects documented across large-scale clinical registries are typically mild and dose-dependent:
Interactions
Older adults aged 65 and over exhibit a significantly higher susceptibility to drug-induced renal decline, advanced orthostatic hypotension, and prolonged amlodipine retention due to age-related hepatic clearance drops. Dosing selection should be highly conservative, typically starting at the lowest available combination strength (5 mg / 160 mg), and closely monitored with routine serum chemistry panels.
