Basics
Clomiphene citrate is a synthetic compound comprising a racemic mixture of two geometric isomers: enclomiphene (the trans-isomer) and zuclomiphene (the cis-isomer). Enclomiphene is the isomer primarily responsible for the ovulatory stimulating effects. In contrast, zuclomiphene, with its significantly longer half-life, contributes more to the peripheral anti-estrogenic effects seen in certain tissues.

Clomiphene is the established first-line treatment for anovulatory infertility and ovulatory dysfunction in women who are seeking pregnancy.
Essential Pre-treatment Screening: To maximize success and minimize risk, all patients must undergo a thorough work-up. Before initiating Clomiphene, potential impediments to conception, such as thyroid disorders, adrenal dysfunction, or significant male factor infertility, must be identified and appropriately managed or excluded.
Mechanism
The primary mechanism involves competitive binding to estrogen receptors, predominantly within the hypothalamus. As a SERM, Clomiphene occupies these receptor sites but does not fully activate them, effectively preventing the hypothalamic tissue from accurately sensing the body’s circulating endogenous estrogen levels. This process creates a “perceived” state of estrogen deficiency.
In response to this perceived lack of estrogen—a condition the body attempts to correct—the hypothalamus increases the pulsatile secretion of Gonadotropin-Releasing Hormone (GnRH). GnRH then travels to the anterior pituitary gland, which in turn releases greater amounts of Follicle-Stimulating Hormone (FSH) and Luteinizing Hormone (LH). The surge in FSH is critical as it directly stimulates the growth and maturation of ovarian follicles. Once a dominant follicle reaches maturity, the natural preovulatory LH surge (which can also be augmented by an administered “trigger shot”) occurs, culminating in follicular rupture and the release of an ovum (ovulation).

Side Effects

Contraindications
Clomiphene is absolutely contraindicated in patients with known pregnancy (Category X), liver disease or history of liver dysfunction, abnormal uterine bleeding of undetermined origin, ovarian cysts not attributable to PCOS, and uncontrolled thyroid or adrenal dysfunction.
Clomiphene citrate is efficiently absorbed following oral administration. Its mixed-isomer composition contributes to a long half-life of 5-7 days, resulting in persistent anti-estrogenic activity on the hypothalamic-pituitary-ovarian axis well after the 5-day course is complete. Excretion primarily occurs via the feces.
Success Rates: The medication is highly effective at inducing ovulation. Clinical data suggest that approximately 80% of appropriately selected anovulatory patients will ovulate successfully within the first 3 months of therapy. However, the clinical pregnancy rate is lower, typically ranging between 10-20% per cycle, influenced by factors such as patient age, underlying cause of infertility, and sperm quality. Due to safety concerns and diminishing returns, therapy is generally limited to a maximum of six total cycles.
Although the FDA-approved indication is strictly for female ovulatory dysfunction, Clomiphene is widely used off-label in men with secondary hypogonadism (low testosterone).
