Mechanism of Action

  • Ketoacidosis: A serious, life-threatening condition that may occur even when blood glucose levels are below 250 mg/dL. If signs of severe metabolic acidosis occur, discontinue Brenzavvy and seek immediate medical evaluation.
  • Lower Limb Amputation: An increased risk has been observed in some trials. Patients should be monitored for signs and symptoms of infection or ulcers of the lower limbs, and Brenzavvy should be discontinued if these complications occur. Routine preventative foot care is important.
  • Volume Depletion (Dehydration): May lead to symptomatic hypotension or acute kidney injury. Monitoring for signs and symptoms of volume depletion is required during therapy.
  • Urosepsis and Pyelonephritis: Serious urinary tract infections that may require hospitalization have been reported. Patients must be evaluated for signs and symptoms of urinary tract infections and treated promptly.
  • Necrotizing Fasciitis of the Perineum (Fournier’s Gangrene): A rare but serious and life-threatening infection. If suspected (pain, tenderness, erythema, or swelling in the genital/perineal area, along with fever), discontinue the drug and institute prompt surgical and antibiotic treatment.
  • Hypoglycemia: The risk of low blood sugar increases when Brenzavvy is used with insulin or an insulin secretagogue (such as a sulfonylurea). A dose reduction of the concomitant medication may be necessary.
  • Genital Mycotic Infections: Monitor patients for these infections and treat appropriately.
  • Fractures: An imbalance in serious fractures was observed in a cardiovascular risk trial.

Interactions

Brenzavvy interacts with various other substances, which can influence its effectiveness and safety profile.

Primary Drug Interactions

  • UGT Enzyme Inducers (e.g., Rifampin): These drugs can significantly reduce bexagliflozin exposure, leading to decreased efficacy. Consideration should be given to adding another antihyperglycemic agent.
  • Insulin and Insulin Secretagogues: Hypoglycemia risk increases. A lower dose of the insulin or secretagogue may be needed.
  • Lithium: Concomitant use may decrease serum lithium concentrations. Serum lithium concentration should be monitored more frequently upon Brenzavvy initiation and discontinuation.

Laboratory Interference

  • Positive Urine Glucose Test: Brenzavvy increases urinary glucose excretion, resulting in a positive test result. Urine glucose tests are not recommended for monitoring glycemic control in patients taking SGLT2 inhibitors.
  • 1,5-Anhydroglucitol (1,5-AG) Assay: Measurements of 1,5-AG are unreliable in assessing glycemic control. Alternative methods should be used.

Contraindications
