Clindamycin vaginal (Cleocin) is a lincosamide antibiotic available as a vaginal cream (2%) or vaginal suppository (100 mg), used to treat bacterial vaginosis in non-pregnant women. It works by inhibiting bacterial protein synthesis. The cream is typically applied once daily at bedtime for 3 or 7 days depending on the product, and the suppository is used once daily at bedtime for 3 days. Common side effects include vulvovaginal itching or burning, vaginal discharge, and headache. As with other clindamycin products, rare but serious Clostridioides difficile-associated diarrhea and colitis have been reported, even with topical or vaginal use, due to some systemic absorption. Avoid tampons, vaginal douches, and other vaginal products during treatment, and avoid intercourse or use a non-latex condom since the product may weaken latex condoms and diaphragms.
